Code 43 USB driver error causing black screen w/ blinking cursor on Win 7 restart

Hi somewhat novice here.  Dell XPS w/ Win 7 ultimate 64, 6 mos old. 
Noticed my 19-in-1 card reader stopped working and has a code 43 error recently.  Also, at same time (I think) when I restart computer, Dell splash screen and progress bar load but upon completion instead of booting to windows, I get a black screen with
a blinking cursor.  Can not affect cursor with keyboard.  Have not tried safe or set-up mode yet (f8 or f12).
When I power off and power back on again, everything loads fine but reader still doesn't work/has same error.
Have tried troubleshooter and uninstall/reinstall of said driver to no effect.
